How are a Breast Augmentation
and Scarless Breast Lift Different?
Though the two surgeries are often conflated and referred to simply as “boob jobs,” breast augmentations and lifts are two distinct procedures that, nevertheless, work together to create a beautiful harmony. Dr. J’s approach is marked not only by his skill and artistry but also by his ability to minimize the number, size, and visibility of incisions.
No two procedures are exactly the same, but Dr. J’s technique is often to use an incision that can be employed for both the augmentation and breast lift (mastopexy) procedures. During a breast augmentation procedure with a scarless breast lift, Dr. J uses incisions around the areola’s border. These allow the doctor to perform both the lift and augmentation through the same incision. He may also use a crescent incision which removes a small amount of skin at the top of the breast shaped something like a croissant or a crescent moon. Dr. J then lifts the breast upwards, changing the location of the areola and nipple area with a single incision.
Another type of scarless breast lift is the circumareolar technique, aka the donut incision. The technique involves making circular incisions around the areola complex.
For comparison, anchor incisions (keyhole), which start around the areola, involve three separate scars. Vertical incisions (lollipop technique) also start around the areola but they go towards the crease of the breast, naturally leading to a longer and more visible scar.
What Makes an Areola Incision Scarless?
Incisions in the areola complex are far less conspicuous for two reasons. First, the skin of the areola is darker, making incisions harder to see. Moreover, the areola is the most private area of the breast and is therefore hidden by clothing the large majority of the time. With these procedures, scarring should be completely invisible even when wearing a fairly skimpy bikini-style top.
Augmentation Options
The options for breast augmentation with a scarless breast lift are the same as for other breast procedures.
- Saline Implants – These are silicone casings filled with a saltwater solution. Since our bodies are naturally largely comprised of moisture with salt, if a leak or rupture occurs the implant’s filling assimilates easily. On the downside, saline implants are more prone to breakage and leaks and are many times more likely to develop issues with rippling as well as wrinkling and asymmetry (breasts that don’t “match”). The main deal breaker for most patients, however, is that saline implants are harder and look and feel less natural compared to other types of breast augmentation
- Standard Silicone Implants – Silicone gel inside a silicone casing has been the standard for some time for the simple reason that it produces the most natural-looking and feeling breasts. They are available in a wide range of sizes and vary in terms of the viscosity – also referred to as the cohesiveness – of the gel inside.
- Fat Transfer – Most people know that implants are used for breast augmentation but there’s a less well-known option that can produce remarkably natural outcomes in terms of both feeling and appearance. Using unwanted fat obtained via liposuction from elsewhere in the body, purifying it, and then adding it to the breast can be an outstanding option for women who are happy with a modest increase in size – a maximum of one cup size. Candidates for this procedure must have sufficient spare body fat for Dr. J to use in the procedure; very slender patients may not be eligible.
The selection of the right kind of augmentation is a highly complex matter. Indeed, there are enough variations among silicone implants that researchers have proposed creating an algorithm to help doctors make their selections.* Dr. J, however, is an expert on all of the options and a believer in the human touch in matters relating to aesthetics.
